      Authorized Training Provider Application Form

      This Application form relates to participation in IBTA MEA's Authorized Training Provider Program.

      The Program is intended to recognize training partners who meet specific requirements, set forth by IBTA MEA, in support of IBTA's Certified Business Professional (CBP) certification and Certified Young Business Professionals (CYBP). This form consists of this signature page, which will be incorporated in the ATP Agreement by reference, and which collectively define the requirements of the Program and the rights and obligations of the Parties relating to participation in the Program:

      (i)

      CBP Authorized Training Provider Agreement Terms and Conditions and

      (ii)

      CBP Authorized Training Provider Guidelines (the "Program Guidelines") a portion of which may be attached hereto and a portion of which will be provided to Partner in the manner to be specified in the ATP Agreement.
